Receiving provider declaration
This form accompanies a submission to the Education Standards Board to receive transfer of an approved provider.
It should be completed by:
- an individual applicant
- company director or secretary
- public officer
- one other member of a management committee (incorporated association)
- director of a cooperative
- managing partner
- appointee according to rules of a corporation or a government school council.
Once complete, attach the form to the Receiving provider application.
