Under the Education and Care Services National Law 2010 Section 174 requires approved providers to notify certain information about complaints/incidents to the Board within a prescribed time.

Notification forms

Please use Form NL01 for notification of complaints and non-serious incidents and Form SI01 for notification of a serious incident.

These forms may be accessed online from the 'Applications and notifications forms' section of the ACECQA website.

Submit them via the National Quality Agenda IT System (NQA IT System). This is an online tool that approved providers can use to submit applications and notifications, pay invoices and view their provider and service information.
